As a Home Health LPN with Touch of Love Home Health Care Agency LLC, you will build relationships with patients as you visit and provide care to them in the comfort of their own homes. You will admit, discharge and re-certify patients for home health care services, attend to their holistic needs, communicate with physicians, therapists and the care team, and oversee home health aides if needed.
As a Home Health LPN with Touch of Love Home Health Care Agency LLC, you must be a caring, patient and flexible teacher with excellent communication and time management abilities. You must possess independent problem solving abilities, excellent clinical assessment skills and be computer literate. You must also be in good physical shape and have the ability to lift/transfer patients to and from wheelchairs, if necessary. Additional requirements of our Home Health LPN include:
- Associate’s of Science degree in Nursing
- Valid LPN license in state of Virginia
- Minimum 1 year acute care nursing experience, home health experience preferred
- Valid driver’s license and one year clean driving record
- Ability to drive within 30-60 miles daily
- Ability to pass a Level 2 background check and drug screen
- Excellent writing and charting skills
- Methodical and tidy work habits
